Key responsibilities of the Physical Therapist include:

• Perform physical therapy evaluations and treatments based on physician orders/referrals.
• Complete comprehensive evaluations using appropriate assessment tools and procedures.
• Review patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans for patient independence and function.
• Regularly re-assess treatment plan effectiveness and progress towards goals.
• Communicate patient needs and progress to the treatment team, physician, and family members.
• Consult with medical providers regarding patient evaluation, treatment, and discharge.
• Recommend and assist in securing adaptive equipment for patient function improvement.
• Maintain and submit documentation as required.
• Adhere to company policies, standards, and regulatory requirements.
• Supervise Physical Therapist Assistants and Aides in patient care activities.
• Participate in staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist include:

• Valid Physical Therapist license
• Home Health (HH) experience
• Experience with HCHB
