Hospice On Call Visiting Nurse

Hartford Healthcare is seeking a compassionate Hospice On Call Visiting Nurse to provide after-hours and weekend home-based hospice care across our Connecticut network. In this role, you will conduct urgent visits, perform thorough assessments, manage pain and symptoms, and coordinate with our interdisciplinary team to ensure seamless, patient-centered end-of-life care. You will educate and support patients and families, use EMR for timely documentation, and triage calls to maintain safety and comfort, while working within a collaborative, growth-focused culture.
