Weekend On Call Visiting Nurse

Hartford Healthcare seeks a compassionate Weekend On Call Visiting Nurse to provide high-quality home-based care to patients across Connecticut. In this part-time role, you will conduct skilled nursing visits, triage calls, and respond to urgent patient needs on weekends. Responsibilities include comprehensive assessments, wound and IV care, medication management, and coordination with physicians and the interdisciplinary team. You’ll document in our EMR, educate patients and families, and contribute to care plans in a supportive, team-focused culture dedicated to excellence and innovation.
