Description: This position will be responsible for the coordination of care for patients who have heart transplants. Responsibilities include immunosuppression monitoring, prophylactic antibiotic management, monitoring for organ rejection, monitoring for infection, and all other routine and typical patient monitoring.

Preferred qualifications include experience in the management of other organ transplant recipients. Experience in managing immunosuppressive therapies is advantageous.

This role coordinates all post-heart transplant patient care across the continuum. The coordinator will work according to provider orders and protocols. The position helps to assess post-heart transplant graft function and general health status. It involves collaboratively determining the need for additional assessment or intervention. The role requires coordination of any changes to medication regimens with appropriate medical staff. Frequent oversight of laboratory results is essential to detect changes that may need immediate action. Effective communication with multidisciplinary team members, patients, and families is critical. The coordinator will facilitate patient activities with consulting departments and physicians. Assisting other nurses and staff to assess, plan, implement, and evaluate care for transplant patients is expected. Assessment of patient and family educational needs and directing communication of these needs to appropriate services is also a requirement.
