Certified Case Manager Position at CarePerks LLC in Tucker, GA

As a Certified Case Behavioral at CarePerks LLC, you will have the opportunity to provide personalized care and support to individuals in need.

Job Responsibilities:

Conduct comprehensive assessments to evaluate the needs of clients
Develop and implement individualized care plans
Coordinate services and resources to meet the client's needs
Advocate for clients to ensure they receive the appropriate care and support
Monitor and evaluate the effectiveness of the care plan

Qualifications:

Valid certification as a Case Manager
Excellent communication and interpersonal skills
Strong organizational and time management skills
Ability to work independently and as part of a team
Compassionate and empathetic attitude
