NURSE PRACTITIONER OR PHYSICIAN ASSISTANT – METRO DENVER

Join a well-established private practice providing primary and preventive care to geriatric patients in skilled nursing and long-term care facilities throughout the Denver area.

You will have the flexibility to determine what time you begin your day based on your assigned facilities and patient needs. The practice offers a high level of independence while ensuring new graduates receive the training and guidance needed to become comfortable and confident in the role.

Responsibilities

• Provide primary and preventive care to patients in assigned facilities
• Develop ongoing relationships with patients and their families
• Collaborate with facility nursing teams and administrators
• Coordinate care with community physicians and other healthcare professionals

Qualifications

• Nurse Practitioner or Physician Assistant
• Colorado license required
• New graduates are encouraged to apply and will be considered
• Previous experience in primary care, hospital medicine, geriatrics, or post-acute care is preferred but not required
