A healthcare facility in Lexington, KY is seeking an experienced Staff Pharmacist to support its Long-Term Acute Care (LTAC) setting. This 13-week contract role offers a fast start and an opportunity to utilize your clinical knowledge and strong communication skills in a critical patient care environment.

Key Responsibilities:

• Maintain adequate medication supplies and conduct timely procurement, including emergency orders.
• Oversee accurate preparation, sterilization, labeling, and dispensing of all medications, ensuring compliance with regulations.
• Conduct inventory, inspect medication storage areas, and manage controlled substances with strict accountability.
• Participate in formulary development, policy reviews, quality assurance, and Pharmacy & Therapeutics Committee meetings.
• Provide patient counseling, medication reconciliation at discharge, and clinical consultation regarding antimicrobial stewardship and pharmacokinetic dosing.
• Train and mentor pharmacy staff, promoting continuous quality improvement and compliance with accreditation standards.
• Perform medication monitoring for contraindications, interactions, dosages, and adverse reactions.

Desired Qualifications and Experience:

• Previous hospital or LTAC pharmacy experience is required.
• Proficiency with EPIC electronic health records.
• Strong communication skills for obtaining medication histories and conducting discharge counseling.
• Clinical expertise in antimicrobial stewardship, infectious disease management, and anticoagulation dosing.
• Basic Life Support (BLS) certification.
