A 13-week contract opportunity is available for a Staff Pharmacist in a Long-Term Acute Care (LTAC) setting located in Lexington, KY. The ideal candidate will have experience in hospital or LTAC pharmacy, and be prepared to start immediately.

Key Responsibilities:

• Maintain adequate drug supply and manage timely procurement processes, including emergency medication orders.
• Conduct annual pharmacy inventory and oversee monthly removal and return of outdated or recalled drugs.
• Prepare, label, and dispense medications accurately, including parenteral and non-parenteral products.
• Document all dispensed medications and maintain records for accountability and compliance with Federal and State pharmacy regulations.
• Conduct monthly inspections of pharmacy and patient care units, ensuring controlled substances are properly managed.
• Participate in the development and updating of the hospital formulary and provide education to healthcare staff.
• Perform medication reconciliation, discharge counseling, and support clinical drug therapy monitoring activities.
• Participate actively in Pharmacy and Therapeutics Committee meetings and quality assurance programs.
• Maintain patient confidentiality and uphold JCAHO accreditation standards.

Qualifications / Desired Experience:

• Prior experience in hospital or LTAC pharmacy settings.
• Basic Life Support (BLS) certification required.
• Proficient in using EPIC systems for medication reconciliation and billing.
• Strong communication skills for gathering medication histories and counseling patients/families.
• Clinical expertise in antimicrobial stewardship, including interpreting culture results (e.g., UTI, pneumonia, SSTI).
• Experience with pharmacokinetic dosing management and anticoagulation dosing.
• Ability to train and develop pharmacy staff and foster positive team dynamics.
