Community Health Navigator for Health Equity (Hybrid)
Health and Welfare Council of Long Island
The Health and Welfare Council of Long Island is looking for a passionate Community Health Worker based in Huntington, NY. In this role, you will lead resource navigation and screening to assist Medicaid members in improving their health outcomes. This position requires a high school diploma or GED and relevant experience in community health settings.
