Job Description All About Home Care is a Houston-based home health care agency. We’ve been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for a Physical Therapist (PT) to help serve patients in the Upper Central North coverage area of Greater Houston , which mainly includes Conroe, Spring, The Woodlands, and surrounding areas. If you have experience in home health and are interested in covering this area, please submit your resume. We use Kinnser software to document patient care. Benefits Flexible schedule Qualifications Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association Currently licensed (licensed physical therapist) in the state of Texas. Two years experience preferred. Ability to exercise initiative and independent judgment. Spanish speaking a plus Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care) Responsibilities Makes the initial assessment visit and generates OASIS documentation the same day of the visit Understands and adheres to establish policies and procedures.
