• Minimum Education: Associate degree

Job Requirements

• Active Registered Nurse (RN) license
• At least 1 year of nursing experience preferred
• Home health, homecare, or case management experience strongly preferred
• HCHB experience is a plus
• Strong communication, organization, and time-management skills
• Ability to work independently while maintaining excellent patient care standards

Responsibilities

• Serve as an RN Case Manager for home health patients
• Provide skilled nursing care and ongoing patient assessments in the home setting
• Coordinate care plans, visits, and patient needs across the service area
• Document care accurately and in a timely manner
• Support quality patient outcomes through education, communication, and follow-up
• Collaborate with the care team to ensure safe, effective, and compassionate care
• Travel between patient visits as needed, with mileage reimbursement according to policy
