RN Care Manager, Emergency Department, 30 Hours (Day/Evening)

Position Summary The RN Care Manager utilizes a collaborative process to assess, plan, implement, monitor, and evaluate the care options and services necessary to meet an individual’s health needs. The role focuses on promoting quality, cost‑effective outcomes by coordinating with providers and facilities across the continuum of services and ensuring resources are used in a timely and value‑driven manner for both the patient and the reimbursement source. Key Responsibilities Assess and develop individualized care plans for patients. Coordinate with multidisciplinary teams to identify and secure appropriate providers and facilities. Monitor care delivery and evaluate outcomes. Advocate for timely and cost‑effective use of resources. Provide case management support to the Emergency Department and overall department goals. Maintain accurate documentation and communication related to patient care plans. Job Requirements Education : ADN required; BSN preferred. Licensure : Must be licensed to practice as a Registered Nurse in Massachusetts. Certification : CCM or related certification within 24 months of hire preferred. Experience : 3–5 years of diversified clinical experience, including care management; 2+ years of use of judgment and decision making in a utilization/case management setting; ED experience preferred. Knowledge of medical/surgical practice patterns, medical terminology, and standard abbreviations. Familiarity with managed care concepts, health care delivery systems and benefit plans. Proficiency in ICD‑9 and CPT nomenclature, and ability to interpret and convert these codes. Strong written and verbal communication, negotiation, analytical, data management, and PC skills. Confidentiality and ethical handling of sensitive information. Schedule & Compensation Position: RN Care Manager – Emergency Department Schedule: 30 hours per week, Day/Evening shift (typically 7:00 a.m.–5:30 p.m. and 11:00 a.m.–9:30 p.m.) Compensation Range: $38.05–$75.33 per hour, based on education, experience, and licensure.
