Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to understand the unique needs of each hospice patient and their families.

• Provide evidence-based individual and group therapy sessions tailored to the emotional challenges faced by families during the hospice experience.

• Work closely with a team of healthcare professionals to create integrated care plans that address the physical, emotional, and spiritual needs of patients.

• Advocate for patients and families by connecting them to community resources, support services, and assistance programs that enhance their quality of life.

• Maintain accurate and timely documentation of client interactions, treatment plans, and progress within our electronic health record system.

• Participate in regular team meetings and training sessions aimed at enhancing your professional skills and collaborative practices.

Patient Population / Client Focus\n

Your primary focus will be on older adults and their families navigating the complexities of hospice care. This often includes individuals facing chronic, terminal illnesses, and may involve managing grief, loss, and emotional distress. Your expertise in supporting this patient population will directly impact their quality of life and make a significant difference during one of the most challenging times in their lives.

Qualifications\n

• A Master’s degree in Social Work from a recognized institution is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.

• Demonstrated experience working with older adult populations, particularly in hospice or end-of-life care settings.

• Strong clinical assessment and diagnostic skills, with the ability to develop effective treatment interventions.

• Excellent communication and interpersonal skills for building trust and rapport with patients and team members.

Compensation and Benefits\n
