$28.00 per hour, As-Needed Position Monday - Friday ONLY, No Nights, No Weekends, No Holidays!Easterseals Florida is seeking a compassionate, professional and dynamic Substitute Nurse (LPN) to join our team in our Winter Park adult day health care center, which serves adults with disabilities, dementia related disorders or other memory impairment conditions. You will perform all health monitoring and nursing services for participants in the program. The Substitute Nurse provides excellent care and customer service in a person-centered setting designed to enhance the quality of life for our participants.What we offer:Professional Training OpportunitiesEmployee Assistance ProgramWork Life BalanceThe Opportunity to Make a Difference in the Community and the Organization A snapshot of what you'll do:Assist in coordinating admissions and discharges, building new participant charts/medical records, ensuring admissions paperwork is completed.Assist in scheduling in-house medical specialty visits, medical/health appointments, and transportation as neededTrack, review, and coordinate quarterly care plan meetings with participants and family/caregiver teamTake, document, and maintain records for each participant, reporting trends outside of parameters, or needing attention from the Center DirectorProvide clinical support, insight, knowledge, and applicable in-service training to program staff.Provide education to participants and family/caregiver team on health and wellness, medical diagnosis', medication, safe living, emergency situations, and resources available.  Qualifications:Graduate of an accredited nursing programCurrent Florida Licensed Practical Nurse LicenseAt least one year of experience working with individuals with disabilities and special needs and/or the aging population and Alzheimer's patientsAbility to assist with the transfer of participants to and from chair or wheelchair and assist clients with activities of daily living (toileting, bathing, ambulating, feeding, changing clothes, etc)CPR and first aid certification a plus, but can be obtained/renewed once hired Easterseals Florida has been rated a Great Place to Work (2020 – 2025) by our employees!
