What Youll Do

As a Home Health Speech Language Pathologist, youll provide patient-centered care that improves communication, swallowing, and overall quality of life.

• 
  Evaluate and treat patients using evidence-based speech therapy practices
  
  



• 
  Develop and adjust individualized care plans based on patient progress
  
  



• 
  Educate patients and families to support independence and long-term outcomes
  
  



• 
  Collaborate with nurses, therapists, and physicians to coordinate care
  
  



• 
  Complete accurate, timely documentation to support clinical and regulatory requirements
  
  



• 
  Participate in care planning, case conferences, and quality improvement initiatives
  
  



• 
  Identify changes in patient condition and adjust treatment accordingly
  
  

What Were Looking For

• 
  Masters Degree in Speech-Language Pathology
  
  



• 
  Active Illinois SLP license (or eligible)
  
  



• 
  Completion of Clinical Fellowship Year (CFY)
  
  



• 
  Minimum 1 year of experience (or strong clinical internship experience)
  
  



• 
  Strong communication, documentation, and time management skills
  
  



• 
  Ability to work independently in a home-based setting
  
  



• 
  Valid drivers license and reliable transportation
  
  

Certifications

• 
  BLS Certification (AHA) required within 6 months of hire (if not already obtained)
  
  

Work Environment & Physical Requirements

• 
  Travel within assigned geographic area
  
  



• 
  Work in patient homes and assisted living settings
  
  



• 
  Ability to lift up to 35 lbs (with assistive equipment/team support as needed)
  
  



• 
  Occasional weekends/holidays based on patient needs
