Position Summary
Wells Health is seeking compassionate and motivated Peer Support Specialists to provide emotional support, advocacy, and recovery-oriented services to residents in long-term care settings. This role focuses on improving quality of life, promoting independence, and supporting behavioral and mental wellness.
Peer Specialists will work collaboratively with facility staff and be supervised by a Physician Assistant and/or MD .
ResponsibilitiesProvide one-on-one peer support to residents.
Assist residents in developing coping strategies and recovery goals
Promote engagement in social, recreational, and wellness activities
Advocate for resident needs within the care team
Maintain appropriate documentation of interactions and progress
Support residents experiencing isolation, depression, or adjustment challenges
Collaborate with interdisciplinary teams (nursing, social work, etc.)
Maintain professional boundaries and confidentiality (HIPAA compliant)
Certified Peer Specialist
Experience working in healthcare, behavioral health, or long-term care settings preferred
Strong communication and interpersonal skills
Ability to work independently across multiple facilities
Must pass background check and health clearance
Bilingual (English/Spanish) a plus.
