Job Summary

Samaritan Services Inc. is seeking a patient and compassionate  Physical Therapist to partner with patients in their homes to promote effective and safe recovery. In this role, you will create personalized Plans of Treatment, coordinate with healthcare teams, and inspire patients toward greater independence.

Key Responsibilities

• Evaluations: Perform initial and ongoing evaluations according to APTA and state standards.

• Treatment Planning: Develop and implement individualized plans including short- and long-term goals, adjusting based on patient progress.

• Patient Care: Administer physical therapy techniques such as manual therapy, gait training, and/or massage.

• Documentation: Maintain accurate medical records and report progress to caregivers, facility staff, and other healthcare professionals.

• Discharge Planning: Develop in-home follow-up programs for patients upon completion of their treatment.

Qualifications

• Education: BS or Master's Degree or DPT in Physical Therapy from an APTA-approved program.

• Licensure: Current Physical Therapy license in the state of New York.

• Certification: Current Basic Life Support (BLS) certification with AED.

• Experience: Minimum of 2 years of experience providing physical therapy in a fully equipped facility.

• Skills: Strong empathy, the ability to motivate and handle patients safely.

   

Work Environment

Care is provided directly in  client homes . Candidates must be comfortable working independently and maintaining strict patient confidentiality.
