Seven Healthcare are seeking a motivated Registered Nurse Case Manager to join our dynamic team in NY. This travel position requires an individual with a strong background in certified home health care, focusing on case management and patient care within a community setting.

- Process orders and update physicians on patient status and changes

- Implement preventive and rehabilitative strategies
- Educate patients and families on disease processes and self-care techniques
- Supervise LPNs to ensure patient health and compliance with care plans
- Participate in the agency's operational and financial objectives
- Engage in professional development and diversity appreciation
- Certified Home Health Care Experience Required
- EPIC or Netsmart experience preferred
- OASIS and Telemetry experience required
- BLS certification
- Current NY RN License
- One year of experience as a clinical nurse in a hospital required, 9 months home care experience preferred
- Strong critical thinking, assessment, and communication skills
- Current NYS driver's license
