Schedule: 8 hours
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40
Requirements:
- Current state license as a Registered Nurse
- Proof of current CPR certification
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years home health experience
- Homecare Homebase software experience
- OASIS experience
- CHPN certification (preferred)
- Current driver's license and reliable transportation with insurance
Roles & Responsibilities:
- Assess patients in home health or hospice settings to identify their physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure continuity and quality of care.
- Educate patients and their families on care, medication, and health management.
- Supervise and provide clinical direction to home health aides to ensure quality service delivery.
- Monitor assigned cases for compliance with third-party payer requirements.
- Provide on-call services as assigned.
