Ensures accurate and timely filing into processing patient medical assistant. Enters data into the Hospice computer system./ppEssential Job Functions:/pulliPrograms phone to Answering Service at end of workday./liliAnswers department telephones, relays messages clearly, and responds to inquiries in a timely and appropriate manner./liliAccurately, sorts and files records and reports in the Patient Medical Record (active and discharge records)./liliCollaborates with LHCSA (HHA) partner agencies to coordinate and secure appropriate Home Health Aide placements for current patients, ensuring timely coverage and continuity of care./liliTimely notifies all required providers of a patients death, including HHA agencies, DME vendors, referral sources, and interdisciplinary team members, in accordance with established procedures./liliOrders supplies/equipment for patients assigned. Requests pick up of rented equipment with vendors for patients discharge as needed./liliVerifies that HHA invoices are accurately entered into the system and ensures all reported hours are correct prior to submission./liliMakes copies and retrieves documents upon request./liliWorks within legal, ethical practice standards to his/her position as well as Calvarys Code of Conduct. Complies with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer./li/ulpRequirements:/pulliHigh School Diploma required.
