Resident Service Coordinator (Full-Time - 35 Hours/Week)

Shift Schedule: Monday-Friday, 9:00 AM - 5:00 PM

As a Resident Service Coordinator, you will be a vital resource for senior tenants, providing individual case management and dynamic group programming. You will play a critical role in helping formerly unhoused individuals successfully transition back into the community, ensuring they have the support, benefits, and advocacy needed to maintain long-term housing stability.

What You'll Be Doing

Under the supervision of the Supervisor of Residential Services, you will manage a caseload of senior tenants, focusing on housing retention, emotional well-being, and social integration.

Key Responsibilities:

• Clinical Assessment & Monitoring: Provide monthly in-home assessments of tenants' social and emotional needs; evaluate functioning, monitor risk, and update case plans accordingly.
• Housing Stability & Advocacy: Assist formerly unhoused tenants with community transitions; ensure all tenants maintain the subsidies and benefits required for permanent housing.
• Case Management: Develop and implement comprehensive case plans, including individual and family counseling to resolve tenancy and personal issues.
• Benefit Coordination: Screen for eligibility and assist tenants with applications for appropriate benefits and entitlements.
• Community Engagement: Facilitate tenant participation in group meetings and recreational activities to build informal support networks with family, friends, and neighbors.
• Referrals & Education: Connect tenants to legal aid, primary care, and mental health services; educate residents on their rights, obligations of tenancy, and service availability.
• Risk Mitigation: Consult with supervisors regarding high-risk tenants or those at risk of eviction; report on common problematic themes or patterns within the tenant community.

What You'll Need

• Education: Bachelor's degree (BSW preferred) in Sociology, Gerontology, Psychology, Human Services, or a related field.
• Experience: Minimum of one year of relevant experience is required.
• Technical Skills: Proficiency in Microsoft Office, Google Drive, and electronic case record systems.
• Communication: Excellent interpersonal and multitasking skills; Bilingual English/Spanish is preferred.
