Mosaic Life Care seeks a compassionate Contracted Direct Care Worker to support patients with daily living tasks and promote independence in home or facility settings. Responsibilities include personal care, mobility support, observation and reporting, light housekeeping, and companionship while following individualized care plans. You will collaborate with an interdisciplinary healthcare team within a culture of compassion, innovation, and teamwork.
