About the Position

Primary Care Nurse Practitioner or Physician Assistant

Location: Colorado

Schedule: Monday through Sunday, 7:00 AM - 7:00 PM

Statewide Opportunities

Opportunities are available throughout most counties statewide. Assignments are scheduled based on provider availability and county demand, with opportunities in all but a limited number of restricted counties.

Position Overview

• In home health and wellness assessments for health plan members
• Typical daily volume of 5 to 8 assessments (appointments generally last 45 to 75 minutes)
• Assessments may include history and physical, medication review, vitals, and basic screenings such as finger stick and urine sample
• Providers leave specimen kits as required by the program and complete all documentation on a tablet device
• No prescribing and no ongoing treatment

Required Qualifications

• Active, unrestricted Colorado NP or PA license
• National certification for NP or PA
• NPI required
• Ability to work independently in the field and complete same day documentation
• Candidates must have at least two years of clinical experience within the past five years to qualify

Availability Requirements

• Providers must be available at least two full days per week and include at least one weekday
• Weekend only availability is not accepted

Travel and Lodging

• Travel and lodging support is available based on provider availability and assignment distance
