Job Summary:
Provide comprehensive physical therapy services to residents in a skilled nursing facility, focusing on improving mobility, strength, and overall functional independence. Collaborate with the healthcare team to develop and implement individualized treatment plans.

Responsibilities:

• Assess patients' physical conditions and develop tailored therapy programs.
• Implement therapeutic interventions to promote mobility, strength, and pain management.
• Monitor patient progress and adjust treatment plans accordingly.
• Educate patients and their families on exercises and strategies to support recovery.
• Document clinical findings and treatment outcomes in accordance with facility protocols.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Possess excellent assessment and clinical reasoning skills.
• Demonstrate ability to work effectively within a multidisciplinary team.
• Have prior experience in a skilled nursing or long-term care setting preferred.
