Physical Therapist

Job Description

We are hiring a Physical Therapist for Home Health/Hospice services in Medford, Oregon. This is an on-site contract opportunity with a strong potential for extension based on business needs and performance.

Job Duration: (3+ months, with possibilities of extension)

Shift Schedule: Monday - Friday | 8:00 AM - 4:30 PM

Coverage Area: Jackson County

Key Responsibilities

Perform Physical Therapy visits including:

Start of Care (SOC)
Physical Therapy evaluations
Recertifications
Resumption of Care (ROC)
Supervisory visits

Meet productivity expectations of 30 points per week
Deliver high-quality patient care in home health and hospice settings

Required Qualifications

Doctorate in Physical Therapy
Active professional Physical Therapy certification
Ability to complete occupational health screenings as required

Preferred Skills (Nice to Have)

Prior Home Health experience
Experience with Home Care Homebase EMR
