Title: Care Coordinator - Community Health and Member Outreach

Location: Remote, Must reside near Louisville, KY (Workers will be asked to go in person to staff meetings once a quarter)

Pay: $23.00/hour

Schedule: Monday-Friday, 8:30 AM-5:00 PM EST

Job Type: Initial W2 6-Month Assignment, High potential for extension and permanent conversion to a FTE.

Job Summary

A-Line Staffing is looking for a Care Coordinator to support a leading managed healthcare organization serving members in Kentucky. This remote position focuses on member outreach, care coordination, and preventive health initiatives designed to improve health outcomes and close gaps in care. You will work directly with members by providing education, coordinating services, assisting with appointment scheduling, and supporting preventive care efforts while collaborating with internal teams and healthcare providers.

Key Responsibilities

• Conduct outbound member outreach and follow-up calls to encourage completion of preventive healthcare services and care gap closure initiatives
• Assist members with scheduling preventive care appointments, including well-child visits, postpartum care, A1C testing, and breast cancer screenings
• Educate members on available health plan benefits, community resources, and healthcare services
• Accurately document all member interactions, outreach activities, and follow-up efforts within required systems
• Coordinate with internal care teams and healthcare providers to support member needs and continuity of care
• Support ongoing outreach campaigns and member engagement initiatives as assigned

Qualifications

• High school diploma or equivalent required
• 1-2 years of experience in healthcare, member services, care coordination, provider scheduling, community resources, customer service, or a related field
• Community Health Worker (CHW) certification or previous CHW experience preferred
• Comfortable speaking with members by phone and documenting interactions accurately
• Ability to work independently in a remote environment while managing multiple priorities
• Must reside near Louisville, KY or Lexington, KY and be willing to attend quarterly in-person staff meetings
