About the Role Join a collaborative, fast-paced team dedicated to helping members access essential healthcare and community services. In this mission-driven role, you will directly impact member health outcomes by supporting care coordination and driving preventive health initiatives. Key Responsibilities Member Outreach & Education: Conduct outbound calls and handle inbound inquiries to educate members on their health needs. Care Coordination: Assist in closing preventive care gaps (including well-child visits, postpartum care, A1C testing, and health screenings). Appointment Scheduling: Schedule provider appointments and coordinate follow-up activities, including connecting members with Social Determinants of Health (SDOH) resources. Real-Time Documentation: Accurately chart and perform data entry across multiple systems simultaneously while speaking with members. Requirements & Qualifications Experience: 1–2 years of related experience in healthcare customer service, member outreach, or community-based support. (Note: Standard call-center-only experience without relevant healthcare/customer service skills is not sufficient). Knowledge Base: Familiarity with medical/clinical environments, provider scheduling, and community resources. Technical Skills: Tech-savvy with prior remote work experience and the ability to navigate multiple databases seamlessly. Soft Skills: Highly adaptable and reliable with written and verbal communication skills. Must be able to build rapport quickly. Work History: Demonstrated job longevity and reliability (if listing previous contract roles, please indicate if the contract was successfully completed). Work Environment & Expectations Remote Setup: Must maintain a professional remote work environment with appropriate work attire.| Video Requirements: Cameras are required to be ON for most meetings. Performance Metrics: Success is measured through productivity, quality of documentation, member engagement, and the achievement of outreach goals
