Responsibilities
- Provide direct patient care for geriatric residents according to the care plan
- Administer prescribed medications and document administration accurately
- Monitor wound care needs and implement appropriate nursing interventions
- Oversee CNA colleagues and coordinate care to maintain continuity and safety
- Communicate changes in patient condition to the supervising registered nurse and healthcare team
- Maintain accurate charting and MAR, uphold infection control and patient safety standards
- Deliver care with dignity, respect, and consideration for residents’ preferences and goals

Requirements
- Active Licensed Practical Nurse license in New York State
- BLS certification from the American Heart Association (AHA)
- Minimum of two years of experience in a long-term care setting
- Ability to work 12-hour shifts in a long-term care environment
- Strong critical thinking, independent work capability, and compassionate patient interaction
