Concierge / Membership-Based Primary Care Physician

Location: Menlo Park, California

Overview

An established, physician-led concierge medical practice is seeking a Primary Care Physician to join its collaborative, relationship-focused care model in Menlo Park, CA. This practice emphasizes high-touch, personalized, and longitudinal care, allowing physicians to build meaningful relationships with patients while delivering evidence-based medicine.

Physicians work within a highly coordinated, team-based environment, partnering with clinical staff and specialists to proactively manage patient health and well-being.

Practice Highlights

• Limited patient panel (~100 patients per physician) to enable deeper relationships and personalized care
• Dedicated support team including a Registered Nurse and Patient Care Administrator
• Collaborative access to in-house and external specialists
• Strong emphasis on preventive, longitudinal, and whole-person care
• High level of clinical autonomy
• Flexible scheduling based on patient needs and physician preferences
• Collegial environment with regular communication and collaboration among physicians

Key Responsibilities

• Build and maintain long-term patient relationships
• Diagnose and manage acute and chronic conditions
• Conduct annual wellness visits, routine check-ups, and home visits as needed
• Provide after-hours support via phone for patient care
• Monitor physical and mental health development
• Lead and collaborate with a dedicated care team
• Develop personalized care plans and health reports for patients

Qualifications

Education & Training:

• MD or DO from an accredited medical school
• Residency completed in Internal Medicine, Family Medicine, or Medicine-Pediatrics

Experience:

• Minimum 5 years of clinical practice experience
• Background in longitudinal primary care preferred
• Experience in academic or multidisciplinary settings is a plus

Skills:

• Strong interpersonal and communication skills
• Comfortable with EHR systems and digital tools
• Ability to work effectively in a team-based care model
• Strong organizational and multitasking abilities
