Remote Care Coach - Medicare Stars & Quality
Humana
Humana Inc. seeks a Care Coach to evaluate member needs, coordinate care, and drive interventions that improve health outcomes and member experience across Medicare Advantage populations. You will collaborate with members, caregivers, providers, and internal partners to resolve barriers to care and promote treatment adherence. Training is mandatory for four weeks, 8:30 a.m.–5:00 p.m. ET, with a 40-hour workweek and later 10:00 a.m.–8:00 p.m. ET schedule. #J-18808-Ljbffr
