Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing and long-term care environment.

• Monitor residents' health conditions and report changes to the appropriate healthcare professionals.

• Administer medications and treatments according to physician orders and facility policies.

• Assist with assessments and implement individualized nursing care plans.

• Document nursing care, medication administration, and changes in residents' conditions accurately.

• Collaborate with RNs, physicians, CNAs, and other healthcare team members to support quality resident care.

• Communicate effectively with residents and their families regarding care needs and treatment plans.

• Follow infection control procedures and maintain compliance with HIPAA, state regulations, and facility policies.

• Support residents with long-term care, subacute care, and memory care needs as applicable.

• Promote resident safety, comfort, dignity, and overall well-being.

Qualifications

• Current, active LPN license in the state of employment.

• Knowledge of nursing practices, medication administration, and resident care procedures.

• Strong communication, organizational, and interpersonal skills.

• Ability to recognize changes in residents' conditions and respond appropriately.

• Ability to work collaboratively with a multidisciplinary healthcare team.

• Commitment to maintaining accurate documentation and following facility policies.

• Experience in skilled nursing, long-term care, subacute care, or memory care is preferred.

• Familiarity with PointClickCare (PCC) and electronic medical records is a plus.
