JOB SUMMARY: The RN Care Manager serves as an integral member of the population health team to coordinate patient care across the healthcare continuum to ensure safe, timely, and cost-effective outcomes.

ESSENTIAL FUNCTIONS:

• Works in a manner that supports the mission and vision of the CIMG and performs in accordance with system-wide competencies and behaviors.

• Develops, implements, and manages care management activities aimed at improving population health outcomes and maximizing value-based care initiatives.

• Conducts comprehensive assessments of high risk/high acuity patients, including but not limited to their medical history, medication, treatment plans, and psychosocial needs.

• Collaborates closely with healthcare providers, other CIMG team members, patients, and families to develop and implement individualized care plans.

• Monitors and evaluates patients’ progress, adjusting care plans as needed.

• Works as a team with the Clinical Care Coordinator between the patient and healthcare providers, ensuring seamless communication and care coordination.

• Instructs and educates patients on healthcare provider instructions.

• Provides appropriate disease process education, within their scope of license.

• Oversees patients’ linkage to community resources and social support.

• Promotes patient independence by establishing measurable care goals, teaching and counseling patient and caregivers, reinforcing or adding to their understanding of disease, medications, and self-care skills.

• Maintains up-to-date records and care notes.

• Maintains a comprehensive working knowledge of community resources, payer requirements and network services for target population.

• Visit patients’ homes, when appropriate, to perform medication reconciliation, assessments and more.

• Utilizes data to target opportunities for improvement.

• Designs workflows to meet program goals and metrics.

• Complies with OSHA standards.

• Attends and participates in organizational and departmental meetings as scheduled.

• Ensures confidentiality in accordance with established policies and procedures.

• Performs other duties as assigned.

QUALIFICATIONS:

• Active RN license with the MD Board of Nursing is required.

• Active certification as a Certified Care Manager (CMM) or previous 5 years’ progressive care management experience to include considerable experience in population health and value-based programs is required.

o If applicant does not have CMM, it must be obtained within the 1st year. CIMG will assist with cost.

• Current CPR certification is required.

KNOWLEDGE, SKILLS, AND ABILITIES:

• A passion and commitment to excellence.

• Strong professional level of knowledge and comprehensive clinical assessment skills in the adult population and chronic disease management.

• Effective interpersonal, verbal, and written communication and presentation skills.

• Must be able to establish and maintain effective working relationships with internal and external customers.

• A knowledge and proficiency in the use of metrics and measures in managing programs and services.

• Strong organizational skills to manage multiple projects, issues, staff, and priorities.

• Knowledge of managed care principles, concepts, and standards of practice for care managers.

• Must have working knowledge of electronic medical records and medical terminology.

• Must be able to adapt and be flexible to meet the business needs to include but not limited to changes in schedule and work location.

PHYSICIAL REQUIREMENTS:

Works in an office environment with extended periods of time sitting. Must be able to perform physical activities such as lifting (up to 25 pounds), balancing, walking, stooping, and handling materials regularly. Manual dexterity required for use of computer keyboard and phone required.

TRAVEL REQUIREMENTS:

This position may require local travel to meet operational needs.
