Clinical Case Manager (RN) — Home Office

SCM Associates, Inc.

SCM Associates, Inc. seeks an experienced Case Manager (RN) to assess, plan, coordinate, and monitor care for clients in a home-based setting. You will collaborate with families, physicians, and the care team to ensure quality, cost-effective outcomes and adherence to care plans. Travel to patient appointments is required several times per week.
