Care Manager

Overview CDAC Behavioral Healthcare is looking for someone to join the WISE team in Circuit 1. This program supports parents in Circuit 1 (Escambia, Santa Rosa, Okaloosa and Walton) who are struggling with substance use concerns. Responsibilities Coordinate and assist in addressing all needs of families, including substance use, mental health, medical, financial stability, and overall wellness. Advocate and assist families to meet their needs and work closely with community providers and child welfare providers. Serve all counties of Circuit 1 (Escambia, Santa Rosa, Okaloosa and Walton). Qualifications Clinical skills and experience with substance use and mental health disorders, parenting and family issues, and community resources. Understanding of ROSC (recovery-oriented systems of care) principles, case management and care coordination principles. At least a bachelor’s degree in a social services field with two years of relevant experience; master’s degree preferred. Level II Background Check and a valid driver’s license. Drug‑free workplace compliance and ability to pass drug screening.
