Community Health Navigator & Care Coordinator

Medical Center PA

The Medical Center in Sikeston, MO is seeking a Community Health Worker to bridge clinical care, social services, and patients. You will conduct outreach, coordinate care, and promote health education within a multidisciplinary team. This full-time position focuses on closing care gaps, improving outcomes, and supporting patients in navigating community resources.
