Join a physician-led primary care organization as a Primary Care Physician (MD/DO) focused on value-based Medicare Advantage and relationship-driven care. This role offers manageable patient panels, dedicated care team support, and the opportunity to practice outcome-focused medicine. You will provide comprehensive care to older adults while emphasizing prevention, care coordination, and long-term health improvement.
