Community Health Worker I: Linkage & Outreach
Northwestern Memorial Hospital
Northwestern Medicine is hiring a Community Health Worker I to bridge patients with social services and healthcare resources in the Chicago area. You will conduct outreach, support access to care, and document interactions in the EMR. This entry-level role requires a high school diploma and 2 years of customer service experience; preferred candidates may have CHW certification or related outreach experience.
