Work Environment While this position is remote, you will engage with clients through virtual consultations, phone calls, and collaborative digital platforms. Our work setting prioritizes flexibility and innovation, allowing you to connect with clients effectively while ensuring adherence to ethical practices and professional standards. As part of our hospice center, you will focus on providing compassionate end-of-life care, understanding the complexities of grief and mental health in this sensitive environment. Key Responsibilities Conduct comprehensive assessments of clients’ mental health needs through telehealth platforms, developing personalized treatment plans that align with their goals. Implement evidence-based practices, ensuring that services provided are tailored to individual client circumstances, including therapy sessions and crisis intervention when necessary. Collaborate closely with interdisciplinary teams, including nurses, doctors, and therapists, to create integrated care plans that enhance service delivery and client outcomes. Provide support and education to family members, reinforcing their role in the client’s care and addressing any emotional, social, or financial concerns. Document all client interactions and maintain accurate records that comply with legal, ethical, and organizational standards. Patient Population / Client Focus The ideal candidate will work with clients facing diverse challenges, including severe mental health disorders, chronic illnesses, and the emotional difficulties associated with end-of-life care. Our clients include individuals from various backgrounds, often with limited access to mental health services. Thus, a deep understanding of the social determinants of health and cultural competence is essential to provide effective support and advocate for their needs. Qualifications A Bachelor’s Degree in Social Work (BSW) or related field; a Master's degree is preferred. Valid and active LCSW license in Texas. Strong understanding of mental health issues and experience with evidence-based interventions. Excellent communication and interpersonal skills, with a demonstrated ability to build rapport with clients and collaborate effectively within a team. Preferred Experience Experience working in hospice or palliative care settings is highly desirable. Familiarity with telehealth technologies and digital care workflows. A background in working with under-served communities and an understanding of their unique challenges. Compensation and Benefits The salary for this position ranges from USD 70,000 to 85,000 annually, commensurate with experience.
