About the Role:

Paramount Premier Homecare LLC is looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team in Bowmansville, NY. Responsibilities:

Provide direct patient care including wound care, medication administration, and vital sign monitoring

Assess and document patient health status, progress, and changes in condition

Collaborate with RNs, physicians, and care team members to implement individualized care plans

Educate patients and family members on care procedures, medications, and health management

Perform skilled nursing tasks in a home care setting in accordance with physician orders

Maintain accurate and timely clinical documentation and patient records

Ensure patient safety and comfort while promoting independence and dignity

Requirements:

Active Licensed Practical Nurse (LPN) license in the state of New York

Previous home care, long-term care, or clinical nursing experience preferred

Strong knowledge of nursing procedures, infection control, and patient safety protocols

Excellent communication and interpersonal skills for patient and family interaction

Ability to work independently and manage time effectively in a home care environment

Valid driver's license and reliable transportation required

CPR/BLS certification preferred
