ISOU Flight Registered Nurse (O-5 Billet) Non-Supervisory
U.S. Immigration and Customs Enforcement (ICE)
Summary The ISOU Flight Registered Nurse serves as a Medical Aircrew Member, providing in‑flight, ground, and sea medical care during ICE missions and DHS operations. Responsibilities include emergency response, patient care, and supporting IHSC facilities. The role requires advanced nursing expertise, adaptability in austere environments, frequent travel, and readiness for rapid deployment domestically and globally. Responsibilities Serve as a flight nurse and program manager. Operate as a Medical Aircrew Member and First Responder during ICE removal missions and provide direct patient care for individuals in ICE custody, IHSC clinics, or during DHS missions in accordance with agency procedures, nursing guidelines, and/or clinical protocols. Handle complex and unusual professional tasks without close guidance in remote and austere conditions. Operate in various aircraft types and ground vehicles, including sea vessels in inclement weather. Provide direct patient care to migrants aboard cutters, small boats, fixed and rotary‑wing aircraft during Alien Migrant Interdiction Operations. Maintain frequent contact with individuals outside the department and high‑level personnel, including diplomatic representatives. Represent U.S. government as an ICE health official when overseas and with other federal agencies, military, and law enforcement. Travel at least 50% of the time and be available for short or long‑term deployments with short notice. Perform duties above the clinical nurse intermediate level in professional nursing. Serve as a 24/7 on‑call medical consultant to IAO for weeks as needed. Provide care to patients with complex physical, mental, or social needs. Develop and implement comprehensive plans to meet patient needs. Manage ISOU specific program objectives, ensuring sustainability and growth. Advise local and departmental officials on policy development, reporting, problem analysis, and procedure design. Perform specialized or complex tasks independently, establishing criteria and evaluation efforts. Actively participate in conferences, meetings, and presentations on high‑consequence issues. Serve as a senior staff nurse (secondary duty) in an IHSC facility as needed when flight nurse and program manager duties are met. Performs other program duties and responsibilities as assigned. Remain flexible and adapt to sudden schedule changes, even if it requires canceling leave. Requirements U.S. citizenship. Successful background investigation and drug screen. Male applicants born after 12/31/59 must register with Selective Service. Must be an active USPHS Commissioned Corps Officer or USPHS Call to Active Duty (CAD) candidate. Civilians are not eligible. Adhere to all conditions of employment, including an 18‑ to 24‑month commitment to IHSC. Must not hold a current Assignment Pay Agreement, or if it exists, it must be fulfilled within 90 days of application submission. Must be able to obtain DHS Secret Clearance and a U.S. Government official passport. Must meet all responsibilities and qualifications by the closing date of the announcement. Physical Demands Lift and carry 50 lbs. of medical equipment up to 100 yards without assistance. Pass FAA Class III Flight Physical for Altitude Physiology Chamber orientation. Don full‑face mask, breathing apparatus, or emergency fire escape mask or hood without difficulty for extended periods. Alternate between kneeling, standing, and walking in confined small‑aircraft spaces. Use phone and computer for extended periods. Sit or stand for extended periods (8 hours or more). Operate in stressful, austere environments with inclement weather and altitudes. Climb ladders and stairs without impediment while carrying medical equipment and personal gear. Work extended hours and remain in aircraft cabins during inclement weather and delays. Have basic swimming skills: swim at least one pool length and tread water for at least 5 minutes. Education and Credentials Actively boarded or new Call to Duty commissioned in the U.S. Public Health Service Commissioned Corps. Pass FAA Class III Flight Physical. Bachelor of Science or Master of Science in nursing from a program accredited by the National League for Nursing Accrediting Commission or the Commission on Collegiate Nursing Education. Current, unrestricted, and valid RN license from any U.S. state, Washington D.C., Puerto Rico, U.S. Virgin Islands, or Guam. Minimum 4 years of direct patient care experience as a registered nurse. Knowledge of the Nurse Practice Act for the licensing state and/or the state in which one practices nursing. Knowledge of a wide range of nursing concepts, principles, and practices. Strict adherence to established policies, guidelines, instructions, procedures, and protocols. Proficiency in Microsoft Office (Word, Excel, PowerPoint). Strong foundational nursing background and skill set. Knowledge of medication administration guidelines and pharmaceutical agents. Comprehensive knowledge of managing non‑compliant patients, special needs populations, and patients with significant deficits in coping skills. Knowledge of safety, infection control, quality assurance, maintenance of patient records, statistical information gathering, and related requirements. Knowledge or experience of nursing management and leadership skills. Knowledge of teaching patient health education skills. Maintain DHS Secret Clearance and a U.S. Government official passport. Maintain minimum flight hours per Official Non‑Aircrew member flight orders. Preferred qualifications (not required): Emergency Department, Intensive Care, Trauma Care, First Responder Care, or Flight Nursing experience. Experience in immigration health system. Correctional health care experience. Certification as Correctional Health Care Professional (CCHP). Knowledge of IHSC policies, NCCHC, and PBNDS requirements. Knowledge of public health preparedness, infection control, and tuberculosis management. Certifications: AMLS, TNCC, PHTLS, TECC, ACLS, PALS, EPC, NREMT, CTRN, CFRN. Department of Defense training in Aerospace Medicine, Flight Nursing (FN), or Critical Care Air Transport Team (CCATT). Prior Immigration/Correctional Flight Nursing experience. Last 2 employment years in direct patient care. ANCC Certification as National Healthcare Disaster Professional (NHDB‑BC). Proficiency in Spanish or another secondary language. Knowledge of writing USPHS individual and unit awards. Additional Information The ISOU Flight Registered Nurse (FN) is a Medical Aircrew Member (MAM) stationed at an ICE facility. Flight nurse and Program Manager duties are primary; senior staff nurse duties at IHSC facility are secondary. The incumbent supports ICE ERO removal missions, serves as sole health care provider and first responder during flights, and initiates emergency treatment when a physician is not present. Comprehensive training is provided by FAA, Department of State, Defense Medical Readiness Training Institute, FEMA, and Department of Interior.
