We CARE for our patients like they are our own FAMILY.Beaumont Home Health is looking for an Occupational Therapist (OT) to join our team!Schedule: Mon - Fri; 8:15 am to 5:00 pmTerritory: Detroit, Warren, Center Line, Hazel Park, FerndaleThe Occupational Therapist I (OT) provides comprehensive skilled care as ordered by the attending physician. The OT I educates patients and their family members and ensures the safety of the patient. The OT I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The company adopts the occupational therapy scope of practice as outlined by the state for its licensed OT personnel.Key ResponsibilitiesProvides skilled occupational therapy in a patient's home as ordered by the attending physicianEvaluates the patient's level of functioning by applying diagnostic and prognostic proceduresDevelops an individualized OT plan of care and makes adjustments as needs changeObserves, records, and reports patient reactions, changes, and progress to attending physician and any appropriate agency staffHelps decrease re-hospitalizations by front loading visits for high risk patientsUses activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activityGuides the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioningArranges the provision of services listed above on an outpatient basis which cannot be given in the patient's homePromotes continuity of care with appropriate admissions, transfers and dischargesCounsels patient and family on the disease/injury and how to manage in all phases of occupational therapyIncorporates patient and family in development of plan of careInstructs patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devicesEnsures patient/family know financial responsibility for needed equipment by utilizing DME department and/or MSWHelps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsListens to patient and family membersOversees and supervises total care of patient provided by COTAsEvaluates the POC based on patient response to treatment/medical condition and adjusts as needed.Communicates with the COTAs on supervisory visit resultsCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationCompletes chart audits at agency site as requestedMaintains active OT license in state of employmentCommunicates with scheduler any changes as neededParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsParticipates in orientation/training of new employeesMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresDrives to patient's primary location per scheduled visit.Daily attendance at assigned visit locations.Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits.Completes and submits all required documentation within specified company requirements.Follows plan of care as permitted within the scope of practice for a Licensed Occupational Therapist, per the state of employment.Other duties as assigned.QualificationsMinimum of one year of experience as an OT in an acute setting is required.Home care experience is desired.Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.Knowledgeable on therapy best practices.Ability to make appropriate occupational therapy judgments.Ability to identify a situation and handle it with the best possible solution.Detail-oriented and observant.Disciplined style of work ethic with the ability to prioritize and be timely.Valid driver's license and auto insurance with your name as a listed driver.Education and CredentialsOccupational Therapist with a current license in the state of employment.Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job with or without reasonable accommodations.KnowledgeReading, Speaking, Writing English Constantly 66%-100%Communications Skills Constantly 66%-100%Computers / PDA Constantly 66%-100%PhysicalWalking - Constantly 66%-100%Bending - Constantly 66%-100%Standing - Constantly 66%-100%Sitting - Constantly 66%-100%Driving - Frequently 34%-66%Lifting up to 50 lbs. with or without assistance - Frequently 34%-66%Stretching/Reaching - Constantly 66%-100%Climbing - Frequently 34%-66%Hand/finger dexterity - Frequently 34%-66%Stooping (bend at waist) - Frequently 34%-66%Sensory ActivitiesDistinguish smell/temperature - Constantly 66%-100%Hearing/Seeing - Constantly 66%-100%Talking in person - Constantly 66%-100%Talking on the telephone - Constantly 66%-100%Hearing in person - Constantly 66%-100%Hearing on the telephone - Constantly 66%-100%Vision for close work - Constantly 66%-100%Other Sensory ActivitiesN/AWe'll help you put your passion for patient care to work.
