Responsibilities Case Manager – Foster Care Program The ideal candidate must reside in the Northwest or Middle Tennessee area of the state: Montgomery, Cheatham, Stewart, Robertson, Sumner, Davidson, and Dickson counties. Universal Health Services, Inc. in partnership with Greystone Foster Care is seeking a dedicated, compassionate Case Manager to add to our growing team. This is a hybrid position that blends remote work with occasional in‑office requirements. The role requires full‑time dedication and flexibility for weekend and evening work. Job Duties / Responsibilities Comply with state regulatory requirements and Greystone Foster Care plan. Ensure that all placement documentation is completed according to established timelines. Complete regular file audits for each child and participate in peer file audits to ensure audit preparedness at all times. Assist with completing foster parent reassessments when necessary. Schedule child/family team meetings at least quarterly and submit the corresponding documentation. Visit each assigned child per policy or as needed. Ensure that annual EPSDT and biannual dental appointments are scheduled and information is provided to DCS. Ensure that outpatient mental health assessments are scheduled for all Level 2 youth and that participation in recommended services occurs. Obtain required consents and releases from appointed guardians for medical appointments and medications. Benefits Highlights Challenging and rewarding work environment. Qualifications Bachelor’s degree in Social Work or related field; master’s degree preferred. One to three years of related experience and/or training, or equivalent combination of education and experience. Valid state driver’s license. Automobile insurance with verification of medical liability.
