Position: Licensed Vocational Nurse (LVN) - Night Shift

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to provide exceptional nursing care within a skilled nursing setting. This role involves assessing, planning, and implementing personalized nursing care for residents under the supervision of a Registered Nurse and the Director of Nursing.

Key Responsibilities

• Assess resident conditions and nursing needs, determining appropriate nursing actions.
• Monitor and assess pain levels, implementing effective pain management techniques.
• Educate residents and families about pain management and care processes.
• Document changes in residents' conditions and nursing interventions accurately in progress notes and care plans.
• Perform and interpret waived testing (point-of-care testing) according to established policies.
• Administer medications while adhering to the eight medication rights to minimize medication errors.
• Notify physicians of any changes in resident conditions and ensure proper follow-up actions are taken.
• Participate in walking rounds and deliver thorough reports to the incoming shift.
• Implement care plans developed by the interdisciplinary care team.
• Respect and dignify residents and their families, addressing psychosocial, cultural, ethnic, and spiritual needs.
• Supervise and support other healthcare team members to ensure the effective delivery of care.
• Exhibit competence in technical and manual skills as per the LVN scope of practice.
• Advocate for resident rights and take proactive measures to protect them.
• Demonstrate creative problem-solving skills and innovative thinking.
• Engage positively with residents, families, colleagues, and management.
• Involve residents and families in their care planning and discharge processes.
• Lead and mentor CNAs, fostering teamwork and excellence within the nursing unit.
• Participate in the orientation of new personnel and assist with performance evaluations as needed.
• Adhere to proper infection control procedures and ensure compliance among staff.
• Maintain composure in emergencies and stressful situations, ensuring a stable environment.
• Follow facility policies and procedures consistently.
• Exhibit a genuine interest in and appreciation for the needs of older adults.
• Self-motivated and capable of organizing workload to complete all tasks efficiently.

Qualifications & Skills

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse license.
• Current Basic Life Support (BLS) certification.
• Ability to read and communicate effectively in English; additional languages preferred.
• Basic computer knowledge; experience with Electronic Health Records preferred.
