RN Case Manager: Home Health & Elder Care Leader
PACE Southeast Michigan
PACE Southeast Michigan is seeking a Registered Nurse Case Manager (RNCM) to provide nursing care for frail elders in the Detroit area. The RNCM will assess needs, create care plans in collaboration with a multidisciplinary team, and ensure quality care delivery. Candidates must have a valid Michigan nursing license and a strong commitment to patient advocacy. This role requires critical thinking, effective communication, and the ability to lead a nursing team. Experience in elderly care is essential.
