RN Case Manager: Elder Care & Home Health Lead
PACE Southeast Michigan
PACE Southeast Michigan is looking for a Registered Nurse Case Manager (RNCM) to deliver comprehensive nursing care to frail elders. The RNCM will assess participants’ health needs, develop care plans, and supervise nursing staff while collaborating with a multidisciplinary team. Qualifications include a current Michigan nursing license, a BSN preferred, and a minimum of one year experience with an elderly population.
