RN Case Manager: Care Coordination & Transitions

Tenet Healthcare is seeking a Registered Nurse (RN) – Case Manager in Detroit to coordinate patient care along the continuum, assess needs, and implement transition plans. You will lead multidisciplinary conferences, review medical necessity, and refer complex cases to social work as needed. The role requires Michigan RN licensure, 2+ years in acute hospital care, and familiarity with InterQual and Tenet Case Management documentation.
