Inpatient RN Case Manager: Care Coordination & Transitions

Henry Ford Hospital - Detroit Main Campus is seeking a Case Manager-RN to coordinate comprehensive patient care and ensure smooth transitions across care levels. You will work to optimize outcomes through strong clinical judgment, teamwork, and effective communication. The role includes six weeks of orientation with a weekend component, on-site shifts, and weekends opportunities.
