RN Case Manager - Acute Care & Care Coordination

You will collaborate with interdisciplinary teams and utilize Cerner to ensure appropriate resource use for optimal recovery and transitions. Minimum qualifications include a Michigan RN license, BLS, and at least 1 year of case management experience in an acute hospital setting.
