Job Description

We are seeking an experienced Outpatient Physical Therapist to join our team. The ideal candidate will work within a multidisciplinary team, focusing on providing comprehensive therapy services to the geriatric population through our Program for All-Inclusive Care for the Elderly (PACE).

Key Responsibilities

• Conduct thorough assessments and develop individualized treatment plans for elderly patients.
• Monitor vital signs and assess pain levels in patients.
• Provide patient and family education regarding home exercise programs and the use of adaptive and assistive devices.
• Identify environmental barriers impacting patient care and recommend solutions.
• Documentation of patient progress and treatment outcomes in the electronic health record system (EPIC).
• Ensure the delivery of quality care in home health and skilled nursing facility settings.

Qualifications

• 1 year of experience in outpatient therapy, specifically with the geriatric population.
• Current BLS certification required.
• Experience with wheelchair seating prescriptions is a plus.
• Proficiency in EPIC charting system preferred.
• Experience in long-term goal setting and time management is highly valued.

Work Schedule

The position requires a 4-day work week, Monday through Thursday, with 8-hour shifts from 08:00 to 16:30. No weekend shifts or on-call responsibilities are required.
