RN Case Manager - Remote

Seeking a full-time RN Case Manager - Remote, who will manage care for members with complex and chronic needs by assessing, developing, implementing, and evaluating care plans while working primarily through telephonic communication. Key responsibilities Ensures members have access to appropriate health services based on their needs Identifies and manages members according to established criteria, facilitating necessary authorizations and referrals Monitors and evaluates the effectiveness of care management plans, making modifications as needed Required qualifications Minimum 5 years of clinical case management experience or equivalent education and experience Associates or Bachelor's degree in Nursing; BSN preferred Current, unrestricted RN license in the applicable state Ability to communicate effectively and collaborate with diverse groups Strong problem-solving, organizational, and time management skills
